If you live here, or are moving into this area, you are welcome to apply to register with our practice.
You will be asked to complete a Registration Form giving your name, address, date of birth, and details of your previous address and doctor. This allows the NHS England to arrange for your medical record to be transferred to us.
You will also be given a practice leaflet which gives you all the information about the GP Practice, the services provided, and the surgery opening times.
There are practical reasons why we might need to be assured that people are who they say they are, or to check where they live. Seeing some form of ID will help to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto us
